Application for Membership to Christian Referral Service

Dear Christian mental healthcare worker - Greetings in the name of our Lord Jesus Christ, and thank you for your desire to become a member of our Christian Referral Service. Our vision is to bring hope - through your much needed services - to a hurting Christian community.

Membership to our referral service is open to individuals who have earned their degree as a: Psychiatrists (M.D., D.O), Psychologists (PhD or PsyD), Social Workers (MSW, LCSW), Licensed Professional Counsellor (LPC), or Licensed Marriage and Family Therapists (LMFT).

Members of our referral service must abide by the principles of Medical Ethics, and agree to the following Statement of Faith and Standards & Principles:

Statement of Faith

• We believe in the Trinity. God is existent in three persons: Father, Son and Holy Spirit.
• We believe in the deity of the Lord Jesus Christ, His virgin birth, sinless life, death and resurrection making atonement for the sins of the world.
• We believe the Holy Spirit’s work is to convict the world of sin or righteousness and to regenerate those who repent of sin and believe in Jesus Christ. He teaches, guides and comforts the believers in Christ.
• We believe that the Bible is the inspired, the only infallible, authoritative Word of God.
• We believe in the present ministry of the Holy Spirit by whose indwelling the Christian is enabled to live a godly life.
• We believe that the second coming of Christ is the hope of the Church and will be personal, bodily, visible and redemptive.
• We believe there are two eternal destinies for men: Heaven and Hell. The former is for the righteous and penitent; the latter for the unrighteous and unpenitent.
• We believe in the spiritual unity of believers in our Lord Jesus.

Standards & Principles

• You must recognize that patients referred to you by christianpsychiatry.com are looking for a psychiatrist who believes that prayer can be a powerful adjunct to their treatment.
• Although your office may not be staffed by all-Christian personnel, your office staff must maintain a respectful attitude toward the Christian faith.
• We will investigate any and all complaints forwarded to us by our referrals. Should it become evident that you have misrepresented yourself, we reserve the right to discontinue your membership without refund.
